* 投稿摘要: Lymphomas are a heterogeneous group of diseases caused by accumulation of malignant cells in the lymphoid tissues. Diagnosis of lymphoid neoplasm sometimes is challenging. Both histopathologic criteria and immunohistochemical characterization of candidate cells are usually employed by pathologists to make an accurate diagnosis. In cases with atypical histopathological features, however, clonality assessment of T-cell receptor (TCR) and immunoglobulin (Ig) provides indispensible information in the differentiation of lymphoid hyperplasia from malignancy and/or B-cell lymphoma from T-cell lymphoma, respectively. The BIOMED-2 PCR protocol has been successfully implicated in our laboratory to support the molecular diagnosis of lymphoid neoplasm. Our experience found an excellent sensitivity (95.7%) and specificity (100%) in the diagnosis of B-cell lymphoma. However, apparently lower clinical sensitivity (50%) was observed in the diagnosis of T-cell lymphoma using primer sets of TCRβ and TCRγ genes designed for western countries (91%). We hypothesized that T cell lymphoma might not be rearranged in our population. Alternatively, sequence variations (polymorphism) of the rearranged gene may compromise the annealing of PCR primers to DNA templates. To address this question, we established the 5’-rapid ampliphication of cDNA ends (5’-RACE), next generation sequencing (NGS), and algorithm for bioinformatic analysis of T-cell lymphoma. The aims of this pilot study are (1) to examine the distribution and frequency of TCRβ VDJ recombination in Taiwanese (2) to identify the impact of sequence variations (polymorphism) of TCRβ on primer annealing (3) to develop superior primer sets for Taiwanese in the diagnosis of T cell malignancy. With help of NGS and bioinformatics, this proposal is designed to develop a molecular diagnosis protocol for T cell lymphoma in Taiwan.
