* 投稿摘要: A rapid identification of the bacteria directly from positive blood culture broths could timely assist clinicians to choose empirical antimicrobial agents. For the purpose, we evaluated identification of microorganisms directly from BacT/ALERT positive non-charcaol culture bottles using the matrix-assisted laser desorption/ionization (MALDI) time-of-fly (TOF), VITEK MS, after an in-house saporin lysis procedure. In this study, a total of 57 monomicrobial and 4 polymicrobial cultures were analyzed. Among monomicrobial cases, we directly identified 80.7% (46/57) of bacteria to species level without subculture, including 20 GPCs, 23 GNBs and 3 yeasts. Additional 8 species were identified by standard procedure with forming colonies and 94.7% (54/57) were correctly identified to species level. Of the 3 discreptant cases, two could be identified to genus level (Corynebacterium and Cryptococcus) and the other failed because of no signals. Concerning the 4 polymicrobial cases, only one case could be partially identified (one out of two species). All consistent results could be obtained in 2 hrs in the same day of positive bottles detecting. Comparing with conventional procedure that might required more than 12 hrs, this new procedure could offer clinicians timely identification. We concluded that direct identification from positive blood culture by using MALDI-TOF with a simple extraction method could timely assist clinicians to deal with monomicrobial bacteremia, resulting in proper antimicrobial therapy.
