* 投稿摘要： Long term lamivudine treatment for chronic hepatitis B virus (HBV) infection leads to emergency of resistant hepatitis B viruses with tyrosine-methionine-aspartate- aspartate (YMDD) mutation on the reverse transcriptase (rt) region of their polymerase gene. HBV treatment guidelines therefore recommend HBV genotypic resistance testing for patients receiving nucleotide RT inhibitors (NtRTIs) who provoke virological failure. We developed a method for simultaneously detecting and genotyping the HBV YMDD region with a novel set of nested PCR primers which are universal in 6 major HBV genotypes. The method is more sensitive (78 IU/mL HBV genome) to detect HBV genotypes than the other two methods, P-S (Polymerase- Surface antigen) region sequencing and multiplex PCR. Our primer set embraces the region from rt163 to rt281 amino acid that also includes most of the second generation drugs’ mutation hotspots. Among 105 patients, only 27 patients were found with specific mutations (11 for M204V, 9 for M204I, 4 for L180M and M204I, 3 for L180M and M204V). All obtained sequences were clearly clustered to predefine their genotypes in both phylogenetic trees of HBV P-S and YMDD region. It means that YMDD motif region (rt163 to rt281) embraced and elucidated by our designed primer set had enough polymorphism for HBV genotyping.
