* 投稿摘要： Aim: To evaluate the response of first and second phase insulin secretion (SPIS) after treatment in drug naïve type 2 diabetes patients. Methods: Total 27 drug naive type 2 diabetes patients were enrolled. They received both frequent sample intravenous glucose tolerance test (FSIGT) and a modified low-graded glucose infusion test (M-LDGGI) before and after treatment. The acute insulin response derived from FSIGT and the insulin-to-glucose slopes during the M-LDGGI test were regarded as first phase insulin secretion (FPIS) and SPIS, respectively. The association of the change of SPIS (δ-SPIS) and various factors were analyzed. Results: SPIS improved significantly after treatment, irrespective to the change of FPIS. Better response of SPIS were found in those were younger (46.9 ± 6.7, 56 ± 9.6 years, p=0.003), with lower body mass index (BMI, 23.7±2.2, 26.4±3.5 kg/m2, p=0.002), higher fasting plasma glucose (FPG, 10.1 ± 3.8, 6.8 ± 1.9 mmol/L, p=0.044), higher hemoglobin A1c (A1C, 11.1 ± 2.3, 8.1 ± 1.5 %, p=0.004), and lower baseline SPIS (median 0.006 vs. 0.082, p=0.0019). In multivariate regression model, the δ-SPIS was correlated negatively to age, BMI and positively to A1C and triglyceride. Conclusions: In drug naïve type 2 diabetes patients, the recoveries of FPIS and SPIS were not synchronized after treatment. The SPIS improved after treatment, especially in those were younger, with lower BMI, higher A1C and triglyceride in the baseline.
