* 投稿摘要： Monocyte could play a major role in the invasion of the pancreas and leads to insulin resistance in type 2 diabetes mellitus（T2DM） patients. On the other hand, Hypoadiponectinemia is reported to be associated with metabolic syndrome. This study was to understand the expression of Monocyte and Adiponectin in T2DM patients. 46 T2DM patients and 39 patients without DM were inclued in this study. T2DM patients were divied to two subgroup; group I (HOMA-IR<2.6) and group II (HOMA-IR≧2.6). Insulin resistance was calculated using the HOMA-IR formula. Result was divided by the average concentration of the control group and the ratio was taken as standardized level and data were analyzed by t test. The standardized level of Monocyte in patients with T2DM (1.18±0.38 fold) showed highly significant differences of control group（**P=0.005）; but the level of Adiponectin in patients with T2DM (0.77±0.23 fold) were lower than control patients, with lowly significant differences of control group（**P=0.003）. The level of subgroup II with Monocyte (1.24±0.42 fold) were higher than control and subgroup I patients (1.08±0.29 fold; **p =0.002). It is similar to before, the level of subgroup II with Adiponectin (0.75±0.23 fold) when compared to control group were lower than subgroup I (0.80±0.25 fold), showed significant of control patients (**p =0.007). The highest of Monocyte accompanied hypoadiponectinemia may cause increase level of HOMA-IR that revealed insulin resistance in patients with T2DM. However, still need further studied to confirm.
