* 投稿摘要： Introduction: Thalassemia, α- or β- thalassemia, is a common hereditary anemia. There are 3~5% people with thalassemia disease in Taiwan. So it’s an important issue to develop a screening criterion with high sensitivity and specificity. Materials and methods: In this study, we try to analyze the relationship between thalassemia, and two items, MCV (Mean corpuscular volume) and HbA2 concentration, with criteria (<80 fl and 3.5% respectively) were adopted to screenα- or β-thalassemia. Results: During 1998 to 2014, there were 3580 patients undergone further genotyping for thalassemia and were divided into three groups: patients with anemia, patients with Hb variant and prenatal diagnosis patients. There were 3.88% thalassemia patients (n= 101) with high MCV level (≥ 80 fl). In terms of α-thalassemia-2, there were 17.24 % patients (n= 31) with high MCV level (≥ 80 fl). By gel electrophoresis and High-performance liquid chromatography (HPLC), there were 16.52% and 0.3 % α- thalassemia patient’s with HbA2 ≥ 3.5% respectively, but there were only 2.89% and 0 % patients respectively with HbA2 ≥ 4%. Conclusions: In conclusion, the level of MCV level is a useful tool to screen thalassemia patients, except α-thalassemia-2 patients. Based on different Hb analysis methods, there are different cutoff values to differentiate various diseases group.
