* 投稿摘要: Oral therapeutic agents that target HCV NS5 replication complex, for example Daclatasvir, has been known to provide high sustained virological response (SVR) in patients with HCV 1b genotype. However, patients harbored NS5A L31M/V or Y93H mutation has been shown getting no benefit from taking the oral therapeutic agents. Analyzing the status of HCV NS5A mutation before taking therapeutic agents, therefore, is urgent and essential. Here, we present a sensitive and quantitative assay, with the principle of pyrosequencing, to detect the viral strains with HCV NS5A mutations. We utilized a specific biotinylated-PCR primers to target NS5A gene and amplified the fragments with one-step RT-PCR. The de novo pyrosequencing assay was performed with QIAGEN PyroMark Q24 system. The operation time of the assay is around 6 hours with 4 hours hand-off, demonstrating the operational efficiency. By analyzing the codons of target sites, not only the presence of L31M/V and Y93H could be distinguished but the quantified information of mutant strains could be provided. The analytic sensitivity of the assay was evaluated with mixtures of wild type and mutant specimens. And the results demonstrated that approximately 5% of mutant could be detected directly, which is way more sensitive than traditional Sanger sequencing. The ability to obtain quantitative results would be very helpful for physicians to evaluate the therapeutic efficacy of HCV oral therapeutic agents.
