* 投稿摘要: Enteric diseases caused by microorganisms represent a significant portion of morbidity and mortality worldwide. Pathogens enter the body through the gastrointestinal tract and the routes of infections typically are spread via contaminated food, water, or contact with vomitus or feces. Each of the causative agents may result in slightly different clinical symptoms, but all cause diarrhea. To evaluate methods for clinical diagnosis of acute enteric infection, this laboratory compared species identification by traditional culture (Biochemical tests and VITEK® MS, Biomerieux) and by qualitative real-time PCR (BD MAXTM Enteric Bacterial Panel assay), detected several specific genes, such as SpaO gene for Salmonella spp., ipaH gene for Shigella spp. or Enteroinvasive E. coli, tuf gene for Campylobacter jejuni and coli, and stx 1a and stx 2a gene for Shiga-toxin producing E. coli. A total of 14 clinical stool samples were tested by qualitative real-time PCR and 6 samples were positive. (Among them, fourteen samples contained simultaneous culture data.) Culture identification for these 14 clinical specimens were as followings: Five were Salmonella spp. positive, one was shiga-toxin producing E. coli positive (shigella sonnei) and eight were no growth for all target pathogens. The comparison of culture identification and qualitative real-time PCR showed all results (14/14) were compatible including genus identification and “positive” or “negative” results. The elapsed time for the two methods was 2 to 5 days for culture identification and 3 hours from real-time PCR. Therefore, it is easy to implement real-time PCR method into routine laboratory work with its high sensitivity, high specificity and shorter turn-around time.
