* 投稿摘要： We report a vancomycin-resistant enterococcus (VRE) that would not grow on blood agar plate and only grow on CHROMagar plate for screening VRE. The VRE isolate was recovered from urine specimen through the VRE screening procedure followed by the identification with MALDI-TOF MS and BioTyper for confirming to be Enterococcus faecium. To further characterize this isolate, it was subcultured and the colonies only appeared around the vancomycin disc on blood agar plate. The growth demonstrated the dependence of vancomycin and revealed to be vancomycin-dependent enterococcus (VDE). In addition, the van genes were detected to be positive in vanA and negative in vanB. Examining the clinical diagnosis, the patient had sepsis with septic shock secondary to urinary tract infection and pneumonia. There were 10 specimens detected to be VRE screening positive from this patient and observed in urine and stool. The organisms isolated were VRE followed by VDE. The MALDI-TOF generating dendrogram showed the similarity between the isolated VDE and the VRE in their genetic background. The duration of vancomycin therapy is 6 days. The broad-spectrum antibiotics used were meroprnan, ciprofloxacin, ceftriaxone, and piperacillin-tazobactam. The finding contributes to the approach of antimicrobial therapy for preventing the infection of multidrug-resistant VDE.
