* 投稿摘要： In Taiwan, More than 60,000/year people with Diabetes mellitus (DM) have gone through the lower limb amputations over the past decade. The DM patient with upper extremity amputation however is extremely rare. The extremity amputation of DM is usually related to the negative mechanism about low density lipoprotein glycation (glycated-LDL) which leads to foam cells depositing in the arterial walls and results in a disorder of peripheral atherosclerosis. A 73 years old female was diagnosed as type 2 diabetes mellitus in 2005 and went through several amputations including right single toe, transmetatarsal in right foot and left lower extremity amputation within five-year period(2005~2010) . The bacteria culture data had then indicated that the patient also encountered serious infection. The old woman was finally received the last amputation of left upper extremity with harshly widespread gangrene on skin in March 2012. Also, the peripheral atherosclerosis had been proved by the microscopy observation of left upper limb brachial artery which demonstrated the significant calcific wall with 95% occlusion of its lumen. According to the gross examination which shows that the extension involvement of necrosis was very close to margin, it seemes to suspect that more proximal regions of arteries such as axially or even subclavian would possibly be involved by the disorder as well. What is more, the severe infection described above would eventually drag the patient’s body into a worse condition. This means that the patient has already faced the very high risk of death as it could demonstrate that the mortality of DM patients with gangrene of upper extremity is highest than the ones caused by any other DM complication. Therefore, related medical care institutions should make lots of effort for this kind of patients by giving more sufficient medical resources and DM educations which could give patients more capabilities to prevent ischemia occurrence in the upper/lower limbs.
