* 投稿摘要：	About 12.7 million cancer cases and 7.6 million cancer deaths are estimated to have occurred in 2008 worldwide. Lung cancer is the leading cause of cancer-related mortality worldwide. In Taiwan, lung cancer was accounted for approximately 25.8% of all cancer death in 2010. Lung cancer has divided to small cell carcinoma and non-small cell carcinoma, metastasis carcinoma. Non-small cell carcinoma has three main sub-types: squamous cell lung carcinoma (SCC), adenocarcinoma (ACA), and large-cell lung carcinoma. Adenocarcinoma accounts for 40% of non-small cell lung cancers. But the lung is a common place for metastasis of tumors from other parts of the body. Some cases were hard to diagnosis. To use immunohistochemistry (IHC) has been well documented as an important ancillary tool in diagnosing lung carcinoma. The histologic-specific tumor markers have been directed at distinguishing primary from metastatic lesions. In this study we use CK7 and CK20 for SCC and thyroid transcription factor 1 (TTF-1) for ACA to help the pathologic diagnosis. To elevate this panel can help us to distinguish ACA and metastasis carcinoma, we collect 18 cases who have lung primary ACA and 6 cases who have metastasis carcinoma. In primary ACA, CK7 is positive approximately 100%, TTF-1 is positive 88.9% and CK20 is negative. In metastasis carcinoma, only CK20 is positive 100%, and then CK7 and TTF-1 are negative. When CK7 and TTF-1 are positive and CK20 is negative in lung cancer, we can easy to diagnosis this is a primary ACA. On the contrary, when CK20 is positive, CK7and TTF-1 are negative, we can know this carcinoma is metastasis. In our study, this panel CK7, CK20 and TTF-1 can help us to distinguish ACA and metastasis carcinoma. Now many studies still work hard to find other histologic-specific tumor markers to assist the difficult diagnosis. A corrective panel can help pathologist to diagnosis difficult cases easier.
