* 投稿摘要: Vibrio vulnificus exists in marine environments. Raw seafood causes high chance of Vibrio vulnificus infection. Furthermore, patients with cirrhosis are vulnerable to Vibrio vulnificus. A 57-year-old male patient with alcoholic cirrhosis suffered from left thigh swelling and pain, fever, and chillness for three days. Pus-like fluid was drained from the swelling thigh. Due to suspicion of Vibrio spp infection, Gram-negative bacilli (GNB) cultures from blood, synovial fluid, and wound were undertaken. Yellow-colonies growing on the thiosulfate-citrate-bile salts-sucrose (TCBS) agar indicated sucrose-fermenting bacteria. However, all results from traditional tests and commercial diagnosis kits were negative. Finally, 16S rDNA analysis of the strain showed 97% sequence homology with the reference strain of sucrose-fermenting Vibrio vulnificus. In this case, sucrose-fermenting Vibrio vulnificus could not be identified by traditional tests but by updated molecular technique accurately. Due to high mortality, quick identification of this bacterium is essential for further management. According to our current observation, when an unknown bacterium showing yellow-colony on the TCBS agar can not be identified by common biochemical tests, the possibility of sucrose-fermenting Vibrio vulnificus should be considered.
