* 投稿摘要: 血球形態自動判讀儀器DM96裝機評估 三軍總醫院 臨床病理科,臺灣 蔡玲矯 潘品靜 黃家苓 Evaluation of automated blood cell morphology analyser DM96 Instrument Tsai,Ling Chaio、Pan,Pin Ching、Hwang, Jia Ling Tri-Service General Hosp, Clinical Pathology Division,Taiwan Abstract Hematological routine examination is a very popular test. When the specimen abnormal or interfering substances existed, the instruments can provide the IP messages for staff to interpreter the results with the analysis of the graphics and the data. But workloads of medical technologists are heavy. In order to improve its quality, we introduced the automated cell morphology analyser DM96 into routine use and evaluate the performance according to lab manual “Evaluation of instruments and devices installation and movement operating manual” to ensure the reliability of the instruments. The assessment methods include: 1. Confirmation of Instrument basic settings 2. Imprecision test 3. Method comparison between DM96 and manual microscopic differential count. Use EE6 statistical software for data analysis. In addition, we refer CLSI H20-A2 to calculate the 99% confidence interval of manual results as tolerance limit. The instrument basic settings were checked by engineer based on installation checklist, and both the instrument settings and the results of the instrument were normal and confirmed by medical technologists. The standard deviation of the three specimens of the precision testing were Band 1.1、0.99、1.23( tolerance limit is 3.0),Seg 2.21、2.15、3.39(4.7),Lym 1.4、1.7、3.19(5.0),Mono 0.57、1.55、1.07(2.0),Eo 0.48、0.52、0.0(1.2),Baso 0.0、0.47、0.0(0.7) respectively. The bias of the comparison results between DM96 and direct microscopy were Neut 0.59、Lym -1.78、Mono 0.84、Eo -0.05、Baso 0.37、Promyelo -0.19、Myelo -0.1、Meta 0.36, and all the bias fell into 99% confident interval. Most of the items correlation coefficients were good but not include Baso. Because specimens with high-value Baso% were difficult to obtain and the counting number of DM96 and direct microscopy were only 100, the difference of r value and slope of Baso% were worse (0.227/7.897). This evaluation we use Liu’s stain for shorter staining time (2.5 minutes). After DM96 on-line staffs response that the recognition of Monocyte and Eosinophil on DM96 screen were not clear. So the stain was replaced to Wright’s stain recommended by manufacture (staining time 9.5 minutes), and re-evaluate the comparison between DM96 and direct microscopy. The result of the assessment was good. We conclude that the evaluation the performance of precision testing and comparison of DM96 while blood smear prepared by Wright’s stain were satisfied.
