* 投稿摘要： 排除Ammonia 檢體內源性干擾之檢測 蔡玲矯、潘品靜、張錦標 三軍總醫院 臨床病理科，臺灣 A study in the Elimination of Intrinsic Interference in Ammonia Samples Tsai,Ling Chaio、Pan,Pin Ching、Chang,Jin Biou Tri-Service General Hospital,Clinical Pathology Division,Taiwan Abstract Patients with severe hepatic or biliary disease often display severe intrinsic icteric interference (manufacturer interference testing, see figure 1). This type of interference usually results in flag signal in the report, and the signal can only be removed by repeating the test or by collecting the sample again. The study design referred to related literature and findings from national studies, and utilized 3% SSA to remove intrinsic interference, thus reducing the rate of returned tests and minimize turnaround time (TAT). The model used in this study was Roche Cobas 6000 c501, the test reagent was Roche Cobas NH3L, and the sample used involved plasma K2EDTA. 114 test samples visually inspected to have level 2+ Icterus were mixed with 3% SSA (Sulfosalicylic acid) in the ratio of 1:1 to remove protein. After centrifuged for 10 minutes, the clear solution on the top layer was removed and tested with NH3 and serum index. Serum index is an index of intrinsic interference interpreted by the Roche machine, and is used to replace human interpretation. It can be used to separately measure icterus, hemolysis, and Lipemia. After statistical testing of the results, 3% SSA was found to have significant effects in eliminating intrinsic interference, and the elimination of icterus index was superior to the elimination of Lipemia and hemolysis index. The icterus index was reduced from I4+ to I0+, the hemolysis index was reduced from H2+ to H1+, and the Lipemia index was reduced from L1+ to L0+. The main interference when testing NH3 in the laboratory comes from icterus. Therefore, utilization of this method can reduce the number of repeat tests or collection of new samples, so TAT can be decreased and quality of testing can be improved.
