* 投稿摘要: The most widely used technologies for HER2 analysis in breast cancer are fluorescence in situ hybridization (FISH) and immunohistochemistry (IHC). FISH is a gold standard for determination of HER2 status in IHC equivocal cases of breast carcinoma. The aim of our study was to evaluate the reliability of computer-assisted automated analysis of HER2 status determination by FISH analysis, compared to conventional microscopic manual analysis. Twenty breast cancer specimens were analyzed for HER2 gene amplification by FISH using FDA approved Path-Vysion Her-2/neu DNA probe kit . Automated analysis was performed using a MetaSystems. All cases had equivocal results on IHC. For automatic analysis, 5 areas of invasive tumor cells in high power field (40 X) were selected by a pathologist (W-C Chen) and analysed by computer software. For manual analysis, 20 tumor cells were counted and scored by two independent observers, including the same pathologists (i.e. total 40). The criteria for result interpretation was based on College of American Pathologists (CAP) guideline. Of 20 cases, one showed equivocal result by manual assay, but amplified by automated analysis. Except the equivocal one, the results of automated analysis and manual assay were all concordant. Eight cases (42.1%) showed gene amplification, 11 (57.9%) showed no amplification. Automated analysis had higher cell-counting number than manual assay ( average 200 and 40, respectively). Compared to manual assay (considered as good standard), automated analysis had a sensitivity of 100% and specificity of 100%. In our small sample study, computer-assisted analysis with Metafer MetaCyte had a high level of consistency compared to conventional microscopic manual method in assessing HER2 status by FISH in breast cancer. Although automated analysis could analyze more cells than manual method within a short period of time, it had the drawback of inability to distinguish tumor cells from normal cells.
