* 投稿摘要： Abstract Introduction: Hepatitis B is the major cause of liver disease in adults whom are older than 40 years in Taiwan, and literature indicates that with hepatitis B virus, persisting inflammation will result in necrosis of the liver tissue, and will result in developing diabetes. The literature also pointed out that insulin resistance is the important features of type 2 diabetes, and the pro-inflammatory hormone Tumor Necrosis Factor-alpha (TNF-α) are the important cause of the occurrence of insulin resistance in type 2 diabetes. In this study, in order to understanding the correlation of pro-inflammatory hormone TNF-α and insulin resistance in hepatitis B; we hope to provide a reference to the health care providers. Materials and methods: Patients are collected and confirmed by the gastroenterologist and endocrinologist with the informed consent. 43 type 2 diabetic patients, 68 patients without hepatitis B and type 2 diabetes are control group, and 68 patients with hepatitis B.Fasting blood glucose, HbA1c, Insulin Ab and TNF-α are analyzed by Roche C501、Perkin Elmer 1470 instrument and Thermo Multiskan Spectrum. Thedata were compared and determined the difference in degree with t test. Result:The level of HbA1c, fasting blood glucose and Insulin Ab in 68 patients with hepatitis B（6.5±1.6％, 103±39 mg/dL, 6.43±1.2 ％）, 43 type 2 diabetic patients（7.0±1.1 ％, 122±32 mg/dL, 6.39±1.3 ％） are higher than those patients without hepatitis B and type 2 diabetes（5.6±0.3 ％, 88±19 mg/dL, 5.73±0.54 ％）, and showing highly significant differences（***P<0.001）. Although there is no obvious significant difference between each group, but the level of TNF-α in hepatitis B（0.048±0.076 ng/mL）and type 2 diabetic patients（0.049±0.105 ng/mL）are higher than control group（0.034±0.099 ng/mL）。 Discussion: According to our study and the association of fasting blood suagr, TNF-α and insulin Ab, in hepatitis B patients, the occurrence of high blood sugar may be due to the expression of pro-inflammatory factor TNF-α.
