* 投稿摘要： Influenza A virus infection is a highly contagious airborne disease caused by a negative-sense RNA virus belonging to the Orthomyxoviridae family. The matrix (M) protein is a major component of the virus particle, involved in virus assembly and budding. The M genes are highly conserved and retained throughout the antigen shift hemagglutinin and neuraminidase in human pandemics. Taiwan CDC’s criterion standard for diagnosing influenza A of severe complicated cases is a M gene-based real-time, reverse-transcription PCR (rRT-PCR), a good sensitive approach with turnaround times of less than 24 hours and viral culture, a process may require 3-10 days from nasopharyngeal or throat samples. During October 2011, a specimen tested negative on the M gene of influenza A rRT-PCR, however, the influenza A (H3N2) virus was detected by culture, indirect immunofluorescence staining and RT-PCR subtying. To investigate the cause of this discrepancy, the M gene of this influenza A virus isolate was sequenced and compared with influenza A (H3N2) virus isolated from Jan. to July 2011. The results showed a single nucleotide substitution (G48T) was present within the probe-binding region of M gene. To be able to reliably detect the putative influenza A variant, an additional probe was applied, whilst retaining the previous probe to allow the continued detection of the circulating influenza A virus. As part of the validation assay for this modified rRT-PCR assay, we tested an additional 37 influenza A (H3N2) virus isolates and clinical specimens from Sep. 2011 to Feb. 2012. By the single nucleotide substitution identification of M gene-based probe, we minimize the risk of diagnostic misses and deposit the gene sequences to GenBank (accession number JN990281).
