* 投稿摘要： Naegleria fowleri, Acanthemeba species and Balamuthia mandrillaris are pathogenic free-living amoebae. Currently, there are about 440 cases with CNS infection caused by these pathogenic free-living amoebas reported worldwide1. Usually, N. fowleri results in acute fatal PAM and it mostly happens among immunocompetent children and young adults. Acanthamoeba spp is different from N. fowleri which mainly causes from chronic disease and immunodeficient. The first PAM case was found in southern Australia in 1965 reported by Fowler and Carter.There are more than 310 reported cases are from the U.S., Europe and Australia . According to medical researches, there are only 10 patients surviving from N. fowler. This case is a 75-year-old man presented a seizure-like attack with conscious loss after taking a bath in a hot spring resort. The PAM was diagnosed based on detecting actively motile trophozoites in CSF using the wet-mount smear and the Liu’s stain. The amebas were further comfirmed by polymerase chain reaction and sequencing, and were most similar to N. fowleri. In spite of giving amphotericin B treatment, the patient still passed away on the 25th days after admission. In Taiwan, the first granulomatous amebic encephalitis (GAE) case caused by Acanthamoeba castellanii was released in 2009, but this case report about PAM caused by N. fowler is the first one.
