* 投稿摘要: Obesity is currently a global epidemic associated with many health hazards. Many studies have shown that the relationship between obesity and visceral fat accumulation has effectes on the mechanisms in adipocyte inflammation. This study examines the characteristics of macrophage infiltration and inflammation biomarkers in adipose tissues and its relationship to insulin resistance among morbidly obese patients. We collected blood and omental visceral fat samples from 30 morbidly obese subjects (BMI > 35 kg/m2). In the blood samples, glucose and insulin levels were determined in order to quantify the insulin resistance using HOMA-IR(≧2.4,p=0.003). The participants were sub-divide into two groups, normal and insulin-resistant. Adipose tissue morphology and pathology were determined by using CD68 antibody immunohistochemistry (IHC) to measure the inflammation of macrophages in the omental tissue. Gene expressions of inflammation biomarkers, such as IL-6R and MMP9, were measured using real-time reverse transcriptase polymerase chain reaction (RT-PCR). In obese subjects, we was found the association between an increase in inflammatory response and insulin-resistance status. Moreover, in IR groups, higher levels of adipocyte size(r=0.921,p=0.001),cytokines IL-6R(p<0.1)and MMP9(p<0.028) were found as well as an increase in macrophage infiltration. These findings suggest that visceral fat accumulation may be linked to higher mRNA levels of IL-6R and MMP9, as well as adipose macrophage infiltration.
