* 投稿摘要：	Background. For assays of HIV infection, both false-negative and false-positive results have profound implications. A 27 years old male was attended to our emergency department with fever, sore throat and leukopenia. Physician reported the patient with risk behavior was suspected HIV viral infection. Methods. HIV Screening test was performed with a 4th generation immunoassay Abbott Architect I1000 ( HIV ½ Ag/Ab Combo) and Roche Diagnostics Elecsys E170 (HIV½ Ag/Ab Combo). To establish HIV infection was used Western blot results. HIV-1 subtypes were determined using nested polymerase chain reactions with multiplex primers and phylogenetic analyses. Results. The discrepancy HIV EIA results were founded between Abbott Architect (reaction: 24.62 S/Co, Positive) and Roche Elecsys (Non-reaction: 0.3 S/Co, Negative).However, the Western blot confirmed results was Positive. The HIV strain was determined subtype B by multiplex PCR and Phylogenetic analyses. Conclusions. The inability to detect HIV during early infection (window period) remains a critical limitation in HIV testing. When diagnosis of HIV infection that consideration contact history risk, risk behavior and prevalence rates. The sample with HIV positive results in screening test that should be subject to additional testing including NAT and Western blot confirmatory testing to determine.
